This form is to be completed and sent to the school office three school days before an educational trip for which you wish your child to be excused from school. It is needed for planning lessons and for evaluation of the proposal as a permissible excuse. A $5.00 fee per student per day for days exceeding three consecutive school days is due. Please include fee with the completed form.
